  • Type-hematogenous, multifocal
  • Underlying or associated diseases

  • Detail the infectious agent

  • Indicate any associated injury is current or old

  • Provide information regarding any draining sinus

  • Report any major osseous defect

Necessary Procedures

  • Amputation-site and laterality

  • Excisional debridement

Other

  • Amputation stump infection-specify site and left/right/bilateral
  • Long-term use of immunosuppression drugs-specify drug and type of encounter (initial, subsequent, sequela)

Why

  • Any known causes or underlying conditions (link with associated diabetes, PVD or malnutrition), current smoker or history of smoking
