Progression

  • When did disease process start
  • Initial versus subsequent visit

Location

  • Native artery or bypass graft, autologous vein bypass graft, nonautologous biological bypass graft
  • Laterality-left, right or bilateral
  • Location-thigh, calf, heel, etc.

Acuity

  • Total or complete occlusion of artery

Name

  • If patient has intermittent claudication or rest pain
  • Any ulceration and/or gangrene

Necessary Procedures

  • Surgical intervention as appropriate-PTA, stent bypass, amputation, etc.

Other

  • If ulcers are present
  • Type and severity of ulcer-limited to breakdown of skin, to fat layer exposed, or necrosis of muscle or necrosis of bone

Why

  • All comorbid conditions, underdosing of medications, tobacco history, etc.
